The newly formed Transitions Service is recruiting a service manager, team manager and social worker for an initial period of 24 months fixed term contract. The work of the transition service will be to support an identified cohort of Haringey children and young people move smoothly and effectively from children’s services to adults’ services.
Many of these young people will already be involved with multiple services, including health, social care and education, so those working in the Transitions Team will need to be able to work with the young person, their parent carers, and professionals across the partnership. Candidates expressing an interest in joining the transition service will need to be skilled, flexible, confident, and committed to achieving agreed targets in the service and be passionate about effective transitions for our young people.
These roles are to support the development of the new service and will involve close partnerships with a range of social care staff including professionals such as EHC case workers, adults and children’s social workers, housing, and educationalist.

Managing a team of social workers and other social care staff within the Transitions Service involves a multifaceted approach to ensure the effective delivery of care and support for young people transitioning to adulthood. This role encompasses overseeing the day-to-day delivery of Care Act assessments, ensuring thorough and precise case management, and guaranteeing that staff members are present at Education, Health and Care Plan (EHCP) annual reviews. Additionally, the role requires attending specified transitions meetings and facilitating parent carers support groups. The aim is to provide a seamless transition experience for young people, coordinating with various stakeholders to ensure that all aspects of their care and developmental needs are addressed. This involves not only the direct supervision of staff but also fostering a collaborative environment where best practices are shared and implemented, ensuring that every young person receives tailored and effective support.
Possessing a Social Work Qualification, such as CQSW/CSS, or Dip SW, along with SWE registration and Enhanced DBS clearance, is foundational for this role, ensuring adherence to professional standards and safeguarding protocols. Coupled with a Management Qualification or DMS equivalent, these credentials prepare an individual for the multifaceted responsibilities of managing a social care team within a statutory framework. Extensive experience in the statutory or voluntary sector, particularly within a local authority, is crucial for understanding the intricacies of adult social care. This background equips the manager with the skills to navigate complex decision-making scenarios, including risk management, which is essential for ensuring the safety and well-being of clients.
A successful candidate must demonstrate motivation and competence in managing and inspiring staff, evidenced by prior experience in supervising or inducting colleagues or students. Effective communication skills, both verbal and written, are necessary to engage with a diverse range of client groups and liaise with external agencies and professionals. This role demands a commitment to the Haringey Vision and Values for Adult Social Care, with the ability to use this framework to inspire and motivate the team. Building and maintaining a strong team spirit is as important as the ability to work collaboratively within a team. Additionally, identifying, assessing, and meeting staff development and training needs are vital for fostering a culture of continuous improvement. Proven organizational skills are essential to meet deadlines and priorities in a fast-paced environment, along with the ability to manage performance and handle conflict or crisis situations effectively. A clear understanding of the care management process and related practice and legal issues underpins all these responsibilities, ensuring that services are delivered effectively and in compliance with legal standards.
