About the role

To work within the newly formed Transitions Team and undertake strengths based, person centred assessments and reviews of transition plans. To work closely with young people and their parent carers in line with the Care Act 2014. Social workers will work to maximise the independence of those being assessed to ensure independence and high aspirations, implement preventative interventions, including working with voluntary and community groups and ensure a strengths-based approach is central to the process.

The newly formed Transitions Service is recruiting a service manager, team manager and social worker for an initial period of 24 months fixed term contract. The work of the transition service will be to support an identified cohort of Haringey children and young people move smoothly and effectively from children’s services to adults’ services.
Many of these young people will already be involved with multiple services, including health, social care and education, so those working in the Transitions Team will need to be able to work with the young person, their parent carers, and professionals across the partnership.  Candidates expressing an interest in joining the transition service will need to be skilled, flexible, confident, and committed to achieving agreed targets in the service and be passionate about effective transitions for our young people.
These roles are to support the development of the new service and will involve close partnerships with a range of social care staff including professionals such as EHC case workers, adults and children’s social workers, housing, and educationalist.

A working knowledge of key legislation within the health and social care sector, alongside robust knowledge of health and social care frameworks, is essential for this role. This expertise allows the manager to navigate complex legal landscapes, ensuring that all practices are compliant with current standards and regulations. By understanding the intricacies of legislation such as the Care Act and safeguarding protocols, the manager can make informed decisions that protect the rights and well-being of service users. Additionally, this deep understanding of health and social care frameworks helps shape effective care delivery, guiding the team in implementing best practices and evidence-based strategies to enhance service quality.
Strong written and communication skills are equally critical, enabling the manager to produce outcome-focused documentation that offers clear analysis and solutions. Effective written communication ensures precision and comprehensiveness in reports, care plans, and other documentation, which are vital for meeting the needs of service users and stakeholders. Furthermore, strong verbal communication skills support engagement and collaboration, fostering a positive working environment and facilitating teamwork. The ability to convey complex information clearly and listen actively to feedback builds trust with service users, families, and external partners. Coupled with strong organizational skills and time management, these competencies ensure the manager can handle the varied and demanding responsibilities of the role efficiently, prioritize tasks, manage workloads, and maintain high standards of service delivery.
