About the role

To support the strategic direction of the service and anticipate changes influencing the services both internally and externally, evaluating risk and opportunities and putting appropriate plans into action. To deliver high performing, cost effective services for children and young people transitioning to adult services, working closely with their parent carers and ensuring the young person’s views and aspirations are fully represented in their plans. To ensure the transition is producing high quality strength-based assessments, in a timely and person-centred way; and the service is performing effectively and delivering on its targets. Leadership role in the development of a staff culture that ensures a professional, collaborative person-centred approach to activity and values strong partnership working and co-production.
Ensure that reasonable care is always taken for the health, safety and welfare of yourself and other persons, and to comply with the policies and procedures relating to health and safety within the service. To seek appropriate support as necessary.

The newly formed Transitions Service is recruiting a service manager, team manager and social worker for an initial period of 24 months fixed term contract. The work of the transition service will be to support an identified cohort of Haringey children and young people move smoothly and effectively from children’s services to adults’ services.
Many of these young people will already be involved with multiple services, including health, social care and education, so those working in the Transitions Team will need to be able to work with the young person, their parent carers, and professionals across the partnership.  Candidates expressing an interest in joining the transition service will need to be skilled, flexible, confident, and committed to achieving agreed targets in the service and be passionate about effective transitions for our young people.
These roles are to support the development of the new service and will involve close partnerships with a range of social care staff including professionals such as EHC case workers, adults and children’s social workers, housing, and educationalist.

Relevant professional qualifications in social care or health services are essential, providing a solid foundation of knowledge and expertise. This role also requires registration with the relevant health professional council or governing body, such as Social Care England, which ensures adherence to professional standards and ethical guidelines. Such credentials affirm the manager’s commitment to maintaining high standards of practice and ongoing professional development.
Being a strong team player is vital, especially with the ability to negotiate or influence change diplomatically. Effective change management is a continuous process in the dynamic field of social care, requiring a manager who can navigate challenges and drive improvements. This involves not only implementing new policies and practices but also fostering a culture of excellence where continuous improvement is the norm. The ability to instigate and manage change effectively ensures that the team can adapt to evolving needs and deliver exceptional care consistently.
